IT Service & Support Person Online Entry Form

Applications are welcomed from IT Directors through to Service Analysts and beyond. Nominations for IT Service & Support Person of the Year may come from yourself, senior or junior colleagues or customers.

For each answer the entrant will be required to submit at least 300 words but no more than 400 words and, where possible, provide supporting evidence such as customer newsletters, advertising, performance data, testimonials and any other material that demonstrates the achievement of service excellence.

The winner will receive a £500 cash prize. Please complete the entry form below.

Applicants cannot save forms online. Therefore we advise that all entrants prepare their answers in Word and paste them into the answer boxes below. It is estimated that it will take up to an hour to fill the online form in.

Terms and Conditions

By registering for the awards and submitting the entry form you agree to abide by these Terms and Conditions. (1) Each entry must be submitted electronically on a fully completed form. (2) The judges decision is final and no correspondence will be entered into prior to the judging. (3) The entry form must be submitted to SDI by noon on the closing date of 18th of March 2009. (4) To the extent permitted by law neither party shall be liable to the other for consequential, indirect, special or examplary damages including but not limited to damages for loss of profits, business or anticipated benefits whether arising under tort, contract, negligence or otherwise whether or not foreseen, reasonably foreseeable or advised of the possibility of such damages. (5) By submitting the entry form you agree that the information given in this entry form is accurate and that you are authorised to disclose it. (6) Should the candidate be short listed, they will be required to attend a 30 minute interview at the SDI offices in May 2009. The winner will be announced at the Awards evening on the 9th of June 2009. (7) SDI are not liable for any costs incurred by applicants in the evaluation and judging process.
